Complete BAI Form 2. Complete school details from KG to current also verified from the current school 3. Birth Certificate with same year registration 4. Medical required from any Govt. Notarised Affidavit with duly signed by the deponents 7. Rupees 3000 registration fee payment receipt - To be paid via PNB QR code - Available on Page 3 of PDF (form Link above) 8. Additional documents for players whose born after 2010 (If the registration not same year) i. Medical required from any Govt. Hospitals in BAI format verified by CMO ii. Vaccination record (Immunization record) or iii. Hospital Discharge record during birth More documents were added to the form, pertaining to age proof as per circular issued by BAI on 12th Dec, 2024.
